PURPOSE:
To provide continuous supervision, care and safety to individual being served; to maximize self-help skills of each
individual in the areas of daily living, recreation, communication, community orientation, socialization,
normalization, medication management, and health; to oversee food preparation and household tasks performed
by the individual; to manage the daily routine of the household and to perform food preparation, cleaning and
other household tasks so that a safe, efficient and pleasant home life is maintained.

QUALIFICATION:
Minimum of 17 years of age or as dictated by licensure.

ESSENTIAL REQUIREMENTS:
· Must pass DHS and/or county background clearance.
· Must have valid driver’s license.
· Must meet organizational driving qualifications.
· Must be able to pass CPR/First Aid training.
· Must be able to pass PCS Medication Administration Training Program.
· Must be able to pass competencies required for programs served.
· Must be able to read, write, communicate, and understand the English language while conducting company business.
· Must maintain confidentiality for individuals served, program, and organization.

WORKING ENVIRONMENT:
The work environment may include residential settings, vehicles, and/or various community settings. The DSP
may encounter emotionally upset people, verbal and physical aggressions, potential exposure to communicable
diseases and/or normal household hazards (i.e. electrical, mechanical, chemical).
ESSENTIAL JOB DUTIES:
CARE/SUPPORT:
· Read, comprehend, and follow each person served’s program related plans and procedures.
· Deliver support to the person served based on principles of person-centered services.
· Ability to adapt to changing demands of the program, routines, needs, schedules.
· Ensure needed supervision of the people served and their routines.
· Monitor and assist people we serve with personal cares as necessary (i.e. bathing, toileting, dressing,
grooming, incontinence, menstrual care, etc.).
· Accountable and responsible in fulfilling person served needs and program expectations.
· Use adaptive equipment according to procedure.
· Assist person served with financial management and maintain personal cash guidelines.
· Respond to and report all incidents according to policy and procedure.
· Follow all policies and procedures regarding positive support strategies.
· Protect people served and report maltreatment per policy.

COOKING/DIETARY:
· Prepare healthy food according to the Center for Disease Control (CDC) while reading/following recipes, menus, and diets as ordered.
· Instruct and assist people we serve in food preparation, clean-up, and sanitary practices.
· Follow safe food storage/handling procedures according to Universal Precautions.
· Feed, assist, and/or use adaptive equipment as needed.
· Model table manners by eating with the people we serve.
· Operate kitchen appliances safely and correctly.
· Wash dishes according to procedure.

SOCIAL ROLES:
· Encourage grooming and dress according to the person’s preferences, cultural considerations and social norms
· Demonstrate professional boundaries and provides positive examples to persons served.
· Recognize unique communication styles of each person served.
· Encourage and support person served differences and values promoting their choices and preferences.
· Promote community inclusion and meaningful activities.
· Protect person served rights and be an advocate for them.
· Recognize, accept, and maintain appropriate professional boundaries.

PERSON CENTERED PLANNING/SERVICE DELIVERY:
· Read, comprehend, and implement each person served’s outcomes as written and as scheduled.
· Comprehend and follow each person served’s Coordinated Services and Supports Plan Addendum
(CSSP-A), Self-Management Plan and Individual Abuse Prevention Plan (IAPP), or other program
information as provided.
· Communicate input on person served programs as necessary.
· Encourage individual choices in person centered planning.

MEDICAL/MEDICATION ADMINISTRATION:
· Seek medical assistance through medical care, 911 or emergency assistance in a medical emergency.
· Monitor and document any unusual behavior and overall medical condition of person served.
· Report health needs, changes, or concerns to supervisor/nursing as needed.
· Administer medications and treatments once trained according to protocol.
· Administer CPR/First Aid when needed.
· Assist with medical appointments as needed.

COMMUNICATION:
· Maintain confidentiality of the people we serve, co-workers, program, and organization in accordance with
all HIPAA/data privacy regulation.
· Ability to recognize potential problems, relay relevant information to other staff from shift to shift (utilize
staff communication book and/or verbally discuss as appropriate).
· Ability to recognize potential problems, communicate concerns, suggestions, and information to
supervisors in an accurate, prompt, and respectful manner.
· Attend all in-services and other staff meetings.
· Obtain an excused absence if unable to attend in-services and other staff meetings. Be responsible for
completing make up training within timelines provided.
· Communicate professionally and courteously with people we serve and their family members, outside
services and providers, all PCS staff, and the general public.
· Build cooperative rapport with other care providers and co-workers.
· Follow on-call protocol in case of emergency.

CHARTING/RECORDKEEPING/DOCUMENTATION:
· Record information accurately and timely.
· Chart daily program information.
· Chart only factual information, not your own opinions or feelings.
· Report and document all safety issues.
· Accurately and consistently follow procedures to punch in and out of web-based timekeeping system.

CLEANING:
· Complete cleaning and follow cleaning checklists on each shift.
· Model and assist person served in cleaning tasks.
· Clean and maintain equipment as necessary.
· Operate all household appliances according to manufacturer's recommendations (do not overload
washers or dryers)

TRANSPORTATION:
· Follow PCS’ policy and procedures and vehicle safety checklist before operating.
· Report all vehicle concerns, accidents, or damages immediately, completing appropriate reports.
· Clean vehicles after use and record mileage log.
· Ensure emergency cell phone is in vehicle before operating.
· Ensure fuel tank is at least 1/2 full when returning vehicle to the program.
· Obtain gas at designated gas station utilizing vehicle gas card and keep receipt.
· Ensure driver and passenger safety at all times including seat belting and wheelchair safety.

SAFETY:
· Ensure emergency phone is working and available.
· Use universal precautions at all times including handwashing, laundry, sharps disposal, protective
equipment, and exposure reporting.
· Use and follow emergency policies and procedures.
· Report abuse of equipment.
· Ensure all equipment, cleaning supplies, and medications are stored safely and securely.
· Report all accidents, incidents, and safety concerns to supervisor immediately.
· Keep all entrances, walkways, and vehicles free of ice, snow and other obstructions.
· Document maintenance needs on “maintenance log”.

MISCELLANEOUS
· Other duties as assigned

Wages: $17-$20/our plus benefits.
